This intake form operates exclusively within your PUBLIC CAPACITY — the legal identity established by the United States government and attached to your government-issued identification. Every name, identification number, and piece of information provided on this form must correspond to your official U.S. government records. Do not provide private tribal trust names, private sector designations, or any identification outside of your United States government identity on this form.

The Two Sectors of the Malachian Empire

PUBLIC SECTOR — Malachian Yamassee Tribe:  Operates under your U.S. government birth name and all associated government-issued identification. This form governs your public sector protection and establishes your legal standing in your public capacity.

PRIVATE SECTOR — Malachian Empire:  Operates exclusively under private tribal trust names. No third-party government names, identifiers, or instruments are used or recognized within the private sector. Private sector matters are conducted through entirely separate and independent processes, and are not the subject of this form.

By completing and submitting this form, you acknowledge that you are acting solely in your public capacity and that all information submitted reflects your lawful United States government identity as a member of the Malachian Yamassee Tribe operating in the public sector.

SECTION 3 — MALACHIAN YAMASSEE TRIBAL MEMBERSHIP & CREDENTIALS

This section records your standing as an enrolled member of the Malachian Yamassee Tribe operating in your public capacity.

⚠ Enrollment Prerequisite

A valid Malachian Yamassee Tribal ID Number is required to proceed with this intake. Tribal IDs from any other nation, band, organization, or private sector designation are not accepted on this form and will not be processed.

If you have not yet completed Malachian Yamassee enrollment and received your official Tribal ID Number, you must do so before submitting this application. Contact the Malachian Empire administrative office to begin the enrollment process.

Example: "Descendant of the Yamassee peoples of the southeastern territories." If unsure, leave blank — we will use standard language.
SECTION 4 — CONTACT & MAILING ADDRESS
Used to identify the County Clerk, County Sheriff, and local court jurisdiction for service.
Used to identify the State AG, Secretary of State, DMV, and Governor's Office for service.
SECTION 5 — PASSPORT-STYLE PHOTO SUBMISSION

Your photo is embedded directly into Article I of your Perpetual Injunctive Notice as your official identification image. It must meet the following standards.

Requirements: Front-facing · Clear background (white or light solid color) · Face clearly visible · High resolution (minimum 600×600 pixels) · JPG or PNG format · No sunglasses, hats, or obstructions. This photo will appear on your official document as a biometric identifier.
SECTION 5B — GOVERNMENT-ISSUED IDENTIFICATION

Provide all U.S. government-issued identification numbers. This information is recorded in your Perpetual Injunctive Notice to establish your official identity record and assert your right to travel. Leave any fields that do not apply blank.

List any additional government-issued identification numbers. Separate multiple entries with a comma.
SOCIAL SECURITY
Your SSN is recorded in your private administrative record and referenced in relevant ecclesiastical and UCC filings. This information is held in strict confidence and used solely for document preparation.
BIRTH CERTIFICATE
Located in the upper right or lower portion of your birth certificate. May also be labeled as File Number, Certificate of Live Birth Number, or State File Number.
The state and county where the birth was registered and the certificate was issued.
SECTION 5C — VEHICLE INFORMATION

Your VIN, plate number, and registration information are referenced in your Perpetual Injunctive Notice to establish your right to travel and protect your vehicle from unlawful search or seizure. Provide information for each vehicle you own or regularly operate.
